When hiring an Indonesian domestic helper in Malaysia, the Malaysian Labour Department (JTK) and Immigration Department require certain employer documents for the visa application. The exact documents required depend on your civil status (single, married, divorced), the purpose of hiring (childcare, elderly care, housekeeping) and the number of foreign domestic helpers you are hiring for your household.

D-2 Document Checklist
As a Malaysian/ PR Employer hiring an Indonesian domestic helper to assist with Elderly care & Housekeeping, please provide the following documents:-
Personal Documents
1. Personal Documents
☑ PASSPORT-SIZE PHOTO (BLUE / WHITE BACKGROUND ONLY) of the Employer. Two (2) pieces.
☑ NRIC of the Employer. One (1) copy.
☑ LATEST UTILITY BILL showing residential address where worker will be employed. Astro and/or internet bills will also be accepted. One (1) copy.
Hiring Documents

☑ PASSPORT-SIZE PHOTO of the Elderly Person. One (1) copy.
☑ DOCTOR’S LETTER FOR PROOF OF ELDERLY CARE for the Elderly Person issued by any Government Hospital, Government Clinic or Specialist Hospital. The Doctor’s Letter should sign off on the medical condition of the Elderly Person to support the Employer's application. The Elderly Person must either be the Employer or his/her Spouse themselves (over 60 years old) OR the Employer's Parent or Parent-in-law only. One (1) original document.
- If the Elderly Person is related to the Employer:
☑ BIRTH CERTIFICATE of the Employer and NRIC OF PARENT showing the Employer's maternal and/or paternal relationship to ELDERLY PERSON(S). One (1) copy each.
- If the Elderly Person is related to the Employer's Spouse:
☑ BIRTH CERTIFICATE of the Spouse and NRIC OF PARENT showing the Spouse's maternal and/or paternal relationship to ELDERLY PERSON(S). One (1) copy each.
☑ PROOF OF LARGE RESIDENCE: House front, house interior and street name signage photos to be submitted as supporting documentation for the Application (see Appendix I for Sample Photos). Photos of the Guardhouse and Condominium Building are also required if the Employer is residing in a condominium. One (1) photo each.
☑ WORK PERMIT and PASSPORT COPY of all existing Domestic Workers employed under the Employer.
Civil Status Documents
3.1. Civil Status Documents (if the Employer is Married)
☑ PASSPORT-SIZE PHOTO (BLUE / WHITE BACKGROUND ONLY) of the Employer's Spouse. Two (2) pieces.
☑ NRIC of the Employer's Spouse. One (1) copy. *For foreign spouse, please provide Passport copy and visa copy in Malaysia
☑ MARRIAGE CERTIFICATE. One (1) copy.
3.2. Civil Status Documents (if the Employer is Divorced)
☑ MARRIAGE CERTIFICATE. One (1) copy.
☑ DIVORCE CERTIFICATE. One (1) copy each.
3.3. Civil Status Documents (if the Employer is Widowed)
☑ MARRIAGE CERTIFICATE. One (1) copy.
☑ DEATH CERTIFICATE of Employer’s spouse. One (1) copy.
Income Documents
4.1. Income Documents (if the Employer is Married & working in Malaysia)
☑ LATEST e-Be Form of the Employer and Spouse, if the Spouse is also working AND
☑ E-FILING statement verifying LHDN’s receipt of e-BE of the Employer and Spouse, if the Spouse is also working AND
☑ If there are outstanding tax payments, additionally provide TAX PAYMENT RECEIPT. One (1) copy each.
OR
☑ LATEST EPF CONTRIBUTION STATEMENT of the Employer and Spouse, if the Spouse is also working AND
☑ LATEST THREE (3) MONTHS’ SALARY SLIPS of the Employer and Spouse, if the Spouse is also working.
4.2. Income Documents (if the Employer is Single/ Divorced/ Widowed & working in Malaysia)
☑ LATEST e-Be Form of the Employer AND
☑ E-FILING statement verifying LHDN’s receipt of e-BE of the Employer AND
☑ If there are outstanding tax payments, additionally provide TAX PAYMENT RECEIPT. One (1) copy each.
OR
☑ LATEST EPF CONTRIBUTION STATEMENT of the Employer AND
☑ LATEST THREE (3) MONTHS’ SALARY SLIPS of the Employer.
4.3. Income Documents (if the Employer is working abroad/ out of Malaysia)
☑ LATEST THREE (3) MONTHS’ SALARY SLIPS certified as true copies by Employer's Company HR AND
☑ EMPLOYMENT VERIFICATION LETTER confirming Employer’s current job at said Company AND
☑ LATEST INLAND REVENUE DEPARTMENT STATEMENT proving the payment of income tax in said country of work.
Appendix I: Sample House Photos
If the Employer resides in a landed house:
1. House Front Photo with visible house number

3. Street Name Signage Photo
If the Employer resides in an apartment/ condominium:
1. House Front Photo with visible apartment number
